Male Menopause: What It Is and How to Manage It

What Is This Guide?

This guide explains what male menopause is, the hormonal changes that cause it, the symptoms men may experience, and the lifestyle and medical interventions — including exercise — that can help manage it. What Is Male Menopause?

The term "male menopause" is widely used but somewhat misleading. Unlike female menopause — which involves a rapid and complete cessation of oestrogen production — the hormonal change men experience is gradual and does not affect all men equally.

The medically recognised term is andropause, or late-onset hypogonadism. It describes the progressive decline in testosterone levels that occurs in men from approximately age 30 onwards, typically becoming noticeable in the 40s and 50s. Testosterone levels in men fall by roughly 1 to 2 percent per year from the age of 30.

While this decline is a normal part of ageing, it becomes clinically significant when testosterone falls to levels that produce symptoms affecting quality of life — a condition known as low T (low testosterone) or testosterone deficiency.


Symptoms of Male Menopause / Andropause

Symptoms vary in severity and not all men will experience all of them. Common symptoms of low testosterone include:

What Causes Testosterone to Decline?

The primary driver is age — the Leydig cells in the testes, which produce testosterone, become less active over time. However, several lifestyle factors accelerate this decline:

  • Excess body fat — adipose tissue converts testosterone to oestrogen via aromatase, reducing free testosterone levels
  • Physical inactivity — sedentary behaviour is associated with lower testosterone and more rapid age-related decline
  • Poor sleep — testosterone production is closely linked to sleep quality; sleep deprivation reduces testosterone significantly
  • Chronic stress — elevated cortisol suppresses testosterone production
  • Poor diet — low intake of zinc, vitamin D and healthy fats is associated with lower testosterone
  • Alcohol — regular heavy alcohol consumption reduces testosterone levels


How Is Low Testosterone Diagnosed?

Diagnosis is made through a blood test measuring total and free testosterone levels, ideally taken in the morning when testosterone is at its daily peak. A testosterone level below 300 nanograms per decilitre (ng/dL) is generally considered low by clinical guidelines, though symptoms and the broader clinical picture are also taken into account.

Regular exercise — and strength training in particular — is one of the most effective natural interventions for supporting testosterone levels and managing the symptoms of andropause.

When to Consider Medical Intervention

Lifestyle changes can meaningfully support testosterone levels, particularly in the early stages of decline or when levels have not fallen significantly. However, for men with confirmed clinical low testosterone and significant symptoms, lifestyle changes alone may be insufficient.
